Verification Form Procedures using Adobe Sign

Once we begin sending out verification forms, you will receive an automated email from Adobe Sign (see example below). Follow these steps to sign your forms:

Step 1

Open the email titled “Signature requested on “Verification Forms…” and click the blue Review and sign button in the email.

Step 2

Click the yellow Click here to sign box on the Teacher Signature line .  You will be prompted to type your name.  Enter your name and click Apply.

Step 3

Repeat these steps for every page by clicking the Next required field button at the top of the page.

Please note: the number of fields will be different depending on the number of classes you teach.

Step 3

After you have signed every page, click the blue Click to Sign button at the yellow of the page.
